All information must be filled so you can be informed of the tournament start time. Once form is submitted you will be redirected to a payment page where we accept paypal payments only. *YOU MUST INCLUDE A TEAM NAME

all sign up forms without payment will not be accepted.
there will be a $10 late fee for all sign-ups after February 27th
sign-up includes all beer for tournament
